5th Grade Rules and Consequences:

 

5th grade behavior management plan:

 

Each student is given a spot on which his or her name and classroom number is printed.  Each spot will consist of four cards of different colors and will be displayed on the behavior board.  If a student fails to follow the classroom rules they are given a warning.  Further violation of rules will result in the movement of their card and the student will receive consequences for their behavior.  All behavior is documented on the card.  This process continues until they have reached a pink card at which time the student is sent to the principal’s office.

 

Classroom rules:

 

1.      Act in a Christian manner

2.    When the teacher is out of the room, remain your seat

3.    Keep hands, feet, and objects to yourself

4.    Be considerate and respectful of yourself and others.

 

Consequences:

 

Any violation of the rules will result in consequences.

 

1.      25 sentences signed by parent (green card)

2.     Write 50 sentences to be signed by parent (yellow card)

3.     100 sentences signed by parent and 5 demerits (orange card)

4.     Go the principal’s office and call home (pink card)

 

Severe Clause:  If a student’s behavior is disruptive to the learning of others he or she can be immediately sent to the principal’s office.

Rewards:

 

If the student remains of their green card for the entire week, he or she will earn a sticker on the behavior incentive chart.  Earning three weeks worth of good behavior will entitle the student to choosing a reward from the reward list.

 

Students also earn incentive stickers for completing all homework assignments in a week.  After the student earns five stickers for all homework completed, he or she can choose a reward from the reward list.

 

 

5th grade reward list:

 

Lunch with the teacher

Sit in the teacher’s chair

Be the teacher’s assistant

Choose from the prize bucket

Homework pass (limit 3 per quarter)

Gum pass

Hat pass

Fan by your desk

15 min. of free time

Do Mad Libs with the class

Teacher brings you a snack

15 min. of Cocoa time

Read to the Class

5th Grade Late Assignment Policy (Per Quarter):

 

 

        1st Late assignment (10% off)                            

        2nd Late assignment (same policy as above)

        3rd Late assignment (1 day late 50% off)    

        4th Late assignment (0 and 5 demerits, the student will still

be required to complete the assignment)

        5th Late assignment (0, 5 demerits, and the student will stay

in from recess/P.E. to complete the assignment

without credit.)
